Many asked about how to appeal for subject failed in the examination. Please see below a sample letter. There is no form to be completed although it is provided in the rules 1986 that appeal is RM100.00 per subject.
Please see below extract from Rules 1986 under 5th Schedule, Part II prescribing the Fees for Examination.
As indicated above, appeal fee is RM100 per subject. Including processing fee, the appeal would be RM100 per subject, added with processing fee of RM50.
In such interpretation, if a candidate were to appeal for 3 subjects, the total fee will be:
RM100 x 3 = RM300,
add
RM50 processing fee
Total = RM350.

Salam sejahtera!
I would like to appeal for the subject D05 Principles and Practice of Marketing which I have failed in the recent result announcement.
I hereby attached a cheque payment of RM150 for the purpose of appeal.

Usually, you can write in for the appeal after the official announcement of your results. You may attach your result slip (letter) as reference for clerical convenience.
As nowadays, all registration and payment are done online via bis.lppeh.gov.my, your payment of appeal fees and which subject to appeal are sorted out online as well.
